Please ensure that you bring along the following documents when you travel for your treatment: |

“Fit to Fly” certificate from your treating doctor / family physician (if applicable) |

Copy/originals of all medical reports, prescriptions and other medication details |
3. |
CD copies of any scans like X-RAY / MRI / CT / ECHO / SONOGRAPHY |
4. |
Referral notes from your treating / referring doctor (if any) |
5. |
10 passport size photographs |
6. |
2 copies of passport along with visa page |
